PCOS is commonly diagnosed using the Rotterdam criteria, which require at least two of the following: irregular ovulation or menstrual cycles; clinical or biochemical signs of hyperandrogenism; polycystic ovaries identified on ultrasound.

Despite established criteria, diagnosis is often delayed by years due to symptom variability and gaps in provider education.
